Personal information

The personal information that is required of you is collected for the primary purpose of providing you with psychological care.

Privacy and confidentiality

All information provided by you is strictly confidential. The only exception to this is in the case of:

  1. It is subpoenaed by court; or
  2. Failure to disclose the information would place you or another person at serious or imminent risk; or
  3. Your prior approval has been obtained

Punctuality

If Chantal Kayem attempts to contact you but is unable to reach you at the scheduled time and your session consequently starts late, the session will still end at its scheduled time.

Cancellations

Appointments reminders will be sent to you via text or email 3 business days prior to your scheduled appointment. A cancellation fee will apply for any cancellations made less than 24 business hours before your scheduled appointment.

Unavailability for Zoom session

If Chantal Kayem contacts you and you are unavailable at the scheduled time, the session will be counted as a completed session and no refund or credit will be given.

Indemnification

You agree to indemnify and hold harmless Chantal Kayem and her employees from any and all claims, demands, suits, expenses, costs, judgments or other charges incurred by you as a result of your choice to participate in sessions with Chantal Kayem. You will not hold Chantal Kayem or her employees responsible for any negligent actions or adverse results, whether known or unknown, that you may incur or suffer as a result of sessions you receive with Chantal Kayem.
